Developing multi-agency data sets and robust information sharing processes is key to enabling integration.
Currently data relating to children and young people with SEND is typically held in multiple different places and can be difficult to access and analyse. On top of this, there are often significant gaps in data collection that can make it even harder to build up a complete picture of SEND across a local area. Multi-agency data sets or ‘data dashboards’ can help overcome these challenges and support effective service planning across a local area.
On an individual level, effective information sharing processes can support better coordination of care, allowing practitioners to access information about past or current assessments, care or treatment, and to work collaboratively with other professionals and the family to plan an individual’s care.
