Pathfinder Information Pack: Preparing for Adulthood

This information pack is for anyone who supports children and young people as they prepare for adulthood.

Preparing for adulthood is a key theme across all elements of the reforms including the Local Offer, 0–25 coordinated assessment and Education Health and Care (EHC) plan, multi-agency working, personal budgets, and joint commissioning. In this resource the need for local areas to develop a joint strategy, the importance of personalising the approach and making sure there are a range of options for young people post 16, are highlighted.

Based on the implementation of the Special Educational Needs and Disability (SEND) reforms, this resource pulls together useful top tips, case studies and links to resources that can been used and adapted in local areas to help them develop their preparing for adulthood approach.
