On Thursday 18th September 2025, CDC ran a virtual National Webinar on Transitions into a Fulfilling Adult Life.
The event was aimed at SEND Leads across Local Authorities and Health, Commissioners, DSCOs, DMOs, DCOs and Parent Carer Forum representatives. We also welcomed attendees at practitioner level who wanted to further their knowledge of transitions.
The webinar was chaired by Anna Gardiner, Deputy Director for Health and Wellbeing at the Council for Disabled Children. The webinar consisted of a presentation from Emma Cowley on supporting young people with SEND into training and employment in Middlesbrough, a presentation from Yasmina and Grace, FLARE members and experts by experience on what makes a good transition, a presentation by James Shutkever, Social Care Programme Lead at CDC, on 'Paving the Way', supporting an autistic young person's successful journey out of children's services into adult life, and a presentation by Linda Jordan from the National Development Team for Inclusion (NDTi), on preparing for adulthood from the earliest years. After each presentation, there was an allocated slot for questions and answers, and time for final questions and reflections at the end of the webinar led by Anna Gardiner.

Please note, the presentation by Yasmina and Grace was not recorded, but their slides are available to view within the shared slide deck.
